WTC Health Program | Responders | How To Apply

To apply to the World Trade Center Health Program, you must fill out an application form and send it to the Program along with the required documentation.
 
There are three steps to joining the program:
 
1)   Determine if you are eligible NYC Responder
 
  • An individual, including current and former Federal employees, who worked or volunteered onsite in rescue, recovery, demolition, debris cleanup or related support services in lower Manhattan (south of Canal St), the Staten Island Landfill, or the barge loading piers;

     
  • A member of the Police Department of New York City (active or retired) or a member of the Port Authority Police of the Port Authority of New York and New Jersey (active or retired) who participated onsite in rescue, recovery, debris cleanup, or related services in lower Manhattan (south of Canal St), including Ground Zero, the Staten Island Landfill, or the barge loading piers;

     
  • An employee of the Office of the Chief Medical Examiner of New York City involved in the examination and handling of human remains from the World Trade Center attacks, or other morgue worker who performed similar post-September 11 functions for such Office staff;

     
  • A worker in the Port Authority Trans-Hudson Corporation Tunnel;

     
  • A vehicle-maintenance worker who was exposed to debris from the former World Trade Center while retrieving, driving, cleaning, repairing, and maintaining vehicles contaminated by airborne toxins from the September 11, 2001, terrorist attacks.
 
2)   Provide documentation
 
Please provide copies of documents that show all of these things:
  • Description of your work or volunteer activity at the former World Trade Center site(s),

     
  • Work or volunteer location at the former World Trade Center site(s), and

     
  • The time period that you were working or volunteering at the site(s).
 
Examples of acceptable supporting documentation include copies of:
 
  • A letter from your employer or union stating that you participated in WTC-related activities on or after September 11, 2001

     
  • Written statement by an organization that confirms their relevant work/volunteer activity and work/volunteer location during the days you stated above.

     
  • Memo books, pay stub, 1099, W-2, W-4, tax return, or other documents that show their name, relevant work activity, and relevant work location during the days you stated above.

     
  • An award or letter given by the local, state, or national government, including military awards or letters that commend your WTC activities

     
  • A letter from the Workers' Compensation Board that reports a paid and/or active worker's compensation claim for your World Trade Center activities

     
  • Timesheets or overtime reports that show the day(s), hours worked, and location(s) of World Trade Center activities

     
  • If you enrolled in the WTC Health Registry, you can submit a copy of Wave 1 survey to document your experience at the WTC area with your enrollment form.
 
3)   Apply to the program
 
You can apply to the Program by either download the form and complete it electronically, or you can print it out and complete it manually. If you complete the form electronically you will still need to print it out and sign before sending back to the Program with appropriate documentation. Services for the WTCHP Survivor program are provided through three WTC Environmental Health Center CCE's and through the National Provider Network.
 
Services for the responder program include medical monitoring and treatment for emergency responders, recovery and clean-up workers, and volunteers who helped at the WTC as well as those who worked at the OCME the Staten Island Land fill and on loading Piers they are provided at CCE' in Manhattan, Long Island Queens and NJ as well as through the National Provider Network.
